
Meet the Team
Dr. Rajni Kalagate, M.D.
Dr. Kalagate graduated from medical school in India, where she received several awards for securing the highest scores among her peers. She completed her residency in Internal Medicine and fellowship training in Rheumatology at the Einstein Medical Center in Philadelphia, PA. She is well-regarded in the Fort Worth community and has been awarded Top Doctor by the Fort Worth and 360 West Magazines multiple times. She has been a Medical Honoree by the Arthritis Foundation.
Dr. Kalagate’s fascination with the immune system since medical school led her to gravitate toward a career in rheumatology. She says, “It is interesting to explore the clinical challenges of diagnosing and managing complex rheumatic illnesses. Recent advances in drug development have played a big role in improving quality of life for our patients. This is an exciting time to be a rheumatologist!”

​Dr. Kalagate lives in Fort Worth with her husband and two children. When she is not working, she enjoys spending time with her family, traveling and photography.




Kasandra Montes
Office Manager
I began my career as a medical assistant in 2016 working closely under a Primary Care Physician. After the Primary Care Physician retired, I joined Dr. Kalagate’s office as Office Manager on April 2025. As a dedicated and compassionate Medical Assistant/Office Manager, I am committed to providing exceptional patient care and administrative support. With a strong foundation in clinical procedures and office management, I strive to create a welcoming and efficient environment for both patients and healthcare providers.

Ariana Hisaw
Physician Assistant
Ariana Hisaw is a Physician Assistant who has been working in Rheumatology with Dr. Rajni Kalagate since 2023. She graduated from Baylor University with her B.S. in Biology many years ago and went on to obtain her Master's in Physician Assistant Studies from the University of Toledo Health Science System in 2007. She returned to Fort Worth and began to work with Dr. W. S. Lorimer III in Family Practice in 2008. When he retired, she moved to Kansas City in 2015 and began to work in Rheumatology. She came back to Fort Worth in 2018 and has been practicing in Family Practice and Rheumatology since then.

When not working, she is doing all things baseball with her husband and three sons. As a family, they are active in their community and church. Occasionally she has the time to read, walk the dog and go hiking.
